Giving for Goodness Grant Application Checklist

Please use this application checklist to create your own application and proposal.

Introductory page with basic information, including:

• Legal name of the organization
• Address and phone number
• Project name (if applicable)
• Executive Director
• Project director (if applicable)
• Contact person: Name, title, and phone number
• Amount requested
• Amount of organization's total annual budget
• Has organization applied to Tom's of Maine in the past? When? Was organization funded?
• Ten-line summary of grant request
• Indicate whether this is a Core Mission Grant, Leadership Grant, or Project Grant.

Proposal narrative, including the following topics:

• History of organization
• Organization's mission statement and vision
• Relevancy to Tom's of Maine's mission statement
• Integration of at least two giving categories
• Community need statement
• Organizational need statement
• Population served
• Description of proposed activities and expected outcomes
• How will fulfillment of request strengthen the organization's ability to reach its larger strategic goals?
• Qualifications of the relevant personnel
• Timetable for conducting and completing request

Attachments:

• Project budget, including anticipated sources of funds (if applicable)
• Organization's current annual budget
• Copy of letter confirming tax-exempt status, tax identification number, or copy of fiscal agent's IRS status
• Supplemental materials, including letters of support (materials are welcome, but not required; please limit.)
